Here you can state the order number of your voucher. And then , following the instructions on the form , you can send the PDL to us. Our technicians will accurately measure and inspect your PDL. If the lamp no longer meets specifications, we will advise on the next steps.

Always make sure when you send us a PDL that you attach the request form to the shipment, otherwise we will not be able to process your shipment.
